Form 74.1A — the application
Sign it in front of a commissioner (swear or affirm it). Do not sign it alone at home.
Form 74.1B — the request to file
Sign and date it (no commissioner needed).
Form 74.1C — the draft certificate
Do not sign it. The court signs this one.
The original will
The court needs the real signed will — not a photocopy. Keep it flat and unstapled from anything new.
Proof of death
The death certificate or funeral director's statement.
The tax payment

Send copies first — then wait 30 days
Use the tracking sheet in this package. You may only file after the 30-day wait. The tracking sheet shows how to count it.
File with the court
Follow the filing steps sheet — your courthouse is 330 University – Toronto (Superior Court of Justice, Estates Office).
Golden rule: every name and date must match your documents exactly. Before you sign anything, put the will and the death certificate beside these forms and compare, letter by letter.
